TERMS & CONDITIONS FOR ABAT COURSE
Important (Please read):
By submitting the form below, I acknowledge, understand and agree that:
1. I have visited the QABA website (https://qababoard.com/), fully read and understood the course policy and what it takes to obtain and maintain the ABAT credential, including the application and renewal processes, should I want to pursue ABAT certification.
2. I need to complete all the training and assignments given. I have to fully attend the online tutorials according to the course schedule. I am also expected to stay throughout the class. I am allowed to miss maximum 2 tutorials and I will have to make up my attendance for the missed class by viewing the recorded session. I understand that I will not be eligible to receive the e-Certificate of Course Completion if I do not comply with the course attendance requirements or miss any additional classes.
3. If I cannot attend the quiz on the published date, then I will need to schedule an individual quiz, and this will be chargeable at the rate of RM80.
4. If I fail the quiz, re-sit is free of charge, at a published date set by SEED. Further re-sits are chargeable at RM80 per session.
5. My course registration is strictly non-refundable once confirmed. In the event I cannot attend the course, I have an option to (a) transfer my seat to another person and inform SEED of the changes, or (b) to postpone my registration to the following intake with a deferment fee of RM 300 if done seven (7) days or more prior to the Course Commencement date. Postponement is allowed one time only. Request to postpone with a lesser notice period is not allowed.
6. I will need to pass the course Quiz as set by the instructor(s) at the end of the training to obtain the Certificate of Course Completion from SEED Autism Services.
7. SEED will not issue partial or incomplete Certificate of Course Completion.
8. This course only includes the training and the course Quiz set by the instructor(s). This course does not include the QABA Board Examination or Supervised Fieldwork to become an ABAT.
9. SEED will not provide a client for me for the purpose of Supervised Fieldwork.
10. The course is conducted in English language and I am expected to possess sufficient English language proficiency to understand the course content and actively participate in discussions to fully benefit from the training.
11. This form has been filled up by the participant and not by a representative.

Mode of Payment

Once the form is submitted, the Company will reserve the seat for you for 48 hours within which payment must be made. If payment is not received within 48 hours, the Company reserves the right to release the seat after sending you an e-mail reminder.

Payment may be made in lumpsum or in two installments. Should you choose the installment method, the first 50% payment shall be made upon registration and balance 50% shall be paid at least 2 days before the Orientation session.

The fee category shall be accorded based on the date payment is made.
