TERMS & CONDITIONS

The SRC Membership Program is valid for a lifetime period from the date of registration. 

Membership fee of RM150 is required upon registration to act as a one-time Security Deposit for all rentals made from the SRC Library. 

Membership is non-transferable.

Members must produce proof of identity (IC/Passport) for first time verification and upon collection of rental items. Information provided during membership registration must match information on proof of identity. Non-matches may result in cancellation of rental and other member privileges until identity can be verified.

Any charges arising due to damaged / missing rental items must be paid by the Member within 2 business days from the time of notification. Failure to pay will result in immediate termination of membership, cancellation of privileges and forfeiture of Membership Fee. 

Termination of membership is not allowed within the first 6 months from date of registration.

Termination after 6 months will incur 10% administrative fee which will be deducted from the membership fee before its refund. Refund is strictly via online banking within 3 business days.

SEED Autism Services reserves the right to amend or withdraw any privileges and the terms and conditions from time to time at its sole discretion without prior notice. 

Information obtained within this form will not be used for any purposes apart from the processing of membership account and will not be shared with any third party organisations.
